CareSign™

Switching

Moving to CareSign from another system.

Bring your directories, your roster, your forms, and your history with you, and pick up each patient where your last system left off.

What comes with you

Your records, from the files your current system exports.

Each import reads a file from your previous system, proposes a match for every column, and shows how each row will land before anything is saved.

Contact directories

Agencies, facilities, pharmacies, suppliers, and labs, with their fax numbers. Referral sources on the roster link to this directory, so the right number is on the chart from day one.

Physicians

Referring and ordering physicians, so every chart that names one finds them. For home health, hospice, and wound care.

Patient or client roster

Rows match an existing chart by your previous system's ID, or by name and date of birth. Importing the same file again updates in place and never clears a field that already holds something.

Prior notes and documents

A ZIP of past notes and PDFs. Files named for the patient and the date are matched and dated for you; anything unmatched waits in an intake queue for the office to place.

Wound histories

Each past measurement becomes an imported measurement on its wound, so healing trends continue instead of starting over.

Your forms

Upload the PDFs you already use, import their fillable fields, and place the rest once. Patient and visit data fill in from then on.

Patients already in care

Pick up where they are, for your kind of agency.

Home health

A patient who is already in care starts as already on service, with a start date and the admission source, so their certification period and its deadlines run from that date instead of a new referral.

Hospice

A patient who is already in care starts as already on service at the benefit period they are in, so the 90, 90, then 60 sequence and the face-to-face windows continue from there.

Private duty

Bring the client list and their documents, add each client's care plan to set the shift task list, and the weekly ADL form starts with the first week of shifts in CareSign.

Wound care

Import each patient's wound measurement history so trajectories continue, bring your referrers in with the directory, and set the weekly rounds once as recurring visits.

How the move goes

Four steps, in the order that works.

  1. 1

    Set up your agency

We create your agency on the right preset. A checklist on your dashboard walks the office through contact details, forms, and visit setup.

  2. 2

    Bring your records

Directories first, then physicians, the roster, and prior documents. Every import previews each row and writes nothing until you choose Import, and a receipt of each one stays on the page.

  3. 3

    Invite your team

Invite one person by email or paste a list and review it before sending. Clinicians install the iPhone or Android app and sign in with two-step verification.

  4. 4

    Move at your pace

Start with one clinician or a few patients while the office checks the workflow, then move the rest of the census when you are ready.

What stays behind

Billing history, payments, and your previous system's internal logs stay where they are. CareSign starts charges from the first visit documented in CareSign. Ask your current vendor for your export files early; if the product has no export for something, ask their support team for your files.

Your records stay yours

Records belong to your agency. Signed notes, forms, and packets download as PDFs, and payroll, mileage, and reports export as CSV, whenever you need them.
